How Do I Apply for CDPAP?

If you have Medicaid and you're declared to require assisted home care, does that mean you qualify for CDPAP? You meet the basic requirements, yes, but there are other steps you must accomplish before you're officially approved for CDPAP benefits. Let's break it down into three steps.

1. Applicant Assessment

CDPAP applicants over 18 must undergo a community health assessment and clinical appointment. The New York Independent Assessor will conduct these, and the assessment will determine if you are eligible for CDPAP.

For applicants under 18, a Physician's Order must be sent to the local social services district. The district will conduct the community health assessment to determine eligibility.

2. Required Documentation

Aside from the Physician's Order, other documents will be required to process the application. These include personal information, medical documents, Medicaid enrollment forms and your Social Security number. CDPAP Guidelines in New York

These are the CDPAP guidelines you must adhere to if you're looking to hire someone as a personal assistant (PA) for yourself, a parent or a child:

  • The patient's legal spouse can't be the care provider.
  • For children under 21, the parents can't be hired and paid to provide care.
  • For children over 21, the parents can be hired and paid as the PA.

  • An immigrant can be hired as a care provider so long as they have their legal documents up to date.
